Blepharoplasty


 

This is a 50YO lady that has excessive skin and fat in the upper eyelids. In the lower eyelids, she has a lot of excessive fat and marked indentions in this area about 1.5cm below the argus marginalis. This goes all the way to the sides of her eyes. This is due to some of the facial fat inferiorly. Above this is herniated fat around the orbital area. I removed the fat from an incision just inside her lower eyelid. I put fat grafts in her lower eyelid area to help fill in the indentions and argus marginalis. I also injected some fat in the flattened cheek area. By doing the upper and lower lid blepharoplasty with fat grafts, we have significantly improved the orbital area for her. Insurance will often pay to do a portion of the procedure because of the excessive skin in the upper eyelids interfering with the upper field of vision.

This is a 45YO lady who complains of excessive puffiness of her lower lids. She also has dark circles under the lower lids. This puffiness is caused by excessive fat in the lower lids.  On the post op photos, you can see where the fat was removed from the pockets of the lower lids. I then added fat in a lower portion of the lower lid area to help fill in the indentions in the lower lids.  This significantly rejuvenates the appearance of the eyes. This procedure is done in the office with IV sedation.

This is a 35-year-old lady who is concerned about the excessive puffiness in the upper and lower lids and the darkness of the lower lids with indentions below the puffiness in the lower lids. She underwent an upper and lower lid blepharoplasty. She also underwent fat grafts to fill in the hollowness in the lower lids. Postop results show much more youthful and vibrant eyes. This is a procedure done in the office.

This is a 63-year-old gentleman who presents with problems with his vision. He also complains of excessive puffiness in his upper lids. He also notes that he gets tired in the afternoons and has trouble holding his eyes open. Prior to surgery, he does have some asymmetry of his eyes. His right eye is lower than the left eye. He has a lot of excessive skin and herniated fat in the upper lids. He underwent a bilateral upper lid blepharoplasty. Now he sees better and looks much more vibrant. Insurance paid for his procedure in its entirety.

This is a 50 year old lady who has perfect upper and lower lids. She underwent an upper and lower lid blepharoplasty. She also underwent fat grafting to the lower lids and a lateral cathopexy. She now looks much brighter, younger, and more vibrant than she did pre-operatively. This is a procedure that insurance will often help pay for.

  

This is a 50YO lady that has lot puffiness around her eyes as well as a lot of wrinkling. This has resulted from excessive sun exposure. She has really blue eyes, but you cannot see them well because of all the puffiness. She underwent an upper and lower lid blepharoplasty and fat grafting to the lower lids. She then underwent a chemical peel. You can see in the before and after results that her eyes have greatly been improved.

 

 

This is a 25YO lady from Enterprise, Alabama. She is interested in getting some cosmetic improvement of her eyes. She has excessive skin and hooding in the upper lids that causes her problems with applying eye makeup. Also, she has some bags in the lower eyelids and she feels this makes her look tired. She also says that weight from the excessive skin in the upper eyelids makes her eyes feel tired and sleepy in the afternoon. The extra weight in the upper eyelids fatigues the small muscle that opens the eye. By undergoing upper and lower lid blepharoplasty, we removed the excessive weight in the upper eyelids and resolved her feeling of tired and sleepiness in the afternoon. Also, we remove the bags from her lower eyelids and she looks much more refreshed and youthful. She still has some excessive pigment in the lower eyelids and we are planning some bleaching procedures that will remove some of the excessive darkness in the lower eyelids.

This is a 55YO lady from Enterprise, Alabama.  She presents for some improvement in the appearance of her eyes.  When I examined her upper lids, she has some extra hooding of the upper lids.  Her worst problem is her lower eyelids.  Looking at her lower lids, she has puffiness as well as a moderate amount of wrinkles in the lower lids.  She also has a marked argus marginalis which is a depressed line that goes from the outside of the eyelid down about 1-1.5cm below the eyelid going toward the inner portion of the eyelid. 

I was able to excise fat pockets in the upper portion of the lower eyelid and then fat grafted the tear trough deformity (argus marginalis area).  I was able to correct the puffiness in the lower lid as well as fill in the loss of tissue in the argus marginalis.  The dark circles that were in the lower eyelids look to be much approved as well as the fine wrinkling.  This procedure can be performed in the office.

Sometimes, insurance will pay for the surgical portion of the upper eyelids.

This is a 50YO lady from Enterprise, Alabama who desired some cosmetic improvement of her eyelids. Her main problem is the excessive puffiness and dark circles in her lower lids. Also, the lower lids are starting to hollow. She underwent a lower lid blepharoplasty and fat grafting to the lower lids. After she healed from the blepharoplasty and fat grafting, we did a chemical peel. You can see how much smoother the lower lids look and the darkness and fine wrinkling has almost been totally eliminated. Her eyes now appear much more youthful and a lot brighter.

 

 

This is a 55YO gentleman that had been getting significant signs of aging around his eyes. He had also lost vision in his upper fields due to the hooding of the upper eyelids. In the lower eyelids, he had marked indentions where the lower eyelid joins the cheek area. He underwent an upper and lower lid blepharoplasty Dothan with fat grafts to the lower eyelids. We were able to fill in the indentions around the lower eyelid area as well as remove the bags from the lower eyelids. This is a 50YO lady that had scarring in her forehead area. She had a transverse scar around the upper nose and upper lid area on the right side as well as one vertically on the left side. She has a transverse one in the right forehead too. I was able to treat these with fat grafting which eradicated the scars.

Fat grafting is a procedure where I remove fat from one area of the body and transfer it to another area. This can be done as an outpatient procedure in the hospital. As you can see in her post op photos, this has greatly improved the scarring on her face.

This is a lady that had extreme puffiness in the upper lids with hooding over quite far laterally.  In the lower lids, she has puffiness from excessive fat and also had a prominent argus marginalis of the lower lids with a tear trough deformity.  She underwent an upper lid blepharoplasty and a lower lid transconjunctival blepharoplasty with fat grafting.  As you can see, her eyes appear much brighter.  On the lower lids the bags and puffiness are gone as well as the sharp indention that starts about ½ “below the margin of the lower lid.  The fat grafting really helped these areas.
