Blepharoplasty Procedures

Blepharoplasty is a surgical procedure done to improve the appearance of one’s eyes. Also, it may improve the vision if redundant skin is obstructing. Insurance will often pay for an upper lid blepharoplasty if there are visual field defects caused by the excess skin. An upper lid blepharoplasty will excise excess skin and fat in the eyelids.

Below is a patient that has had an upper and lower lid blepharoplasty. The upper lids before surgery had a lot of skin hooding and you could not even tell that she had a crease due to the excessive fat that was bulging in her upper lids. I removed the excessive skin and fat from the upper lids and you can now see her that her upper lids look much better, more awake, and more youthful. Eyes that could not be seen before surgery because of the skin folds are now visible, alert, vibrant, and youthful.

Lower Lid Blepharoplasty

Lower lid blepharoplasties are done to remove the puffiness of the lower lids. The goal is to create a smooth transition from the eyelid to the cheek without a trough. Therefore, the newer, modern eyelid operations now are combinations of removal and fat grafting. The fat removed improves the puffy appearance. The fat grafted fills out the tear trough at the lid and cheek junction. This depression or tear trough at the junction of the lower eyelid and cheek is called the argus marginalis.

The accentuation of this results from the aging of midface tissues that by the effect of gravity fall down off the bony rim of the eye socket. This gravitational descent of the cheek skin and fat separates it from the eyelid fat above the rim which has gotten more prominent and puffier because of the aging of the internal eyelid structures. Newer eyelid surgery addresses both common problems with the aging lid, excess skin and the fat maldistribution. Loose lids may be tightened against the globe. Fat removed from the lid, or that taken from other parts of the body may be used to graft the depression.

The patient’s before and after photos nicely demonstrates the improvement that can be obtained from upper and lower eyelid surgery.  The upper eyelid crease is sharply defined by removing excess hooding skin.  Puffiness in the lower eyelids is resolved by removing the excess fat.  The fat may then be placed over the bony rim below the eye to create a youthful seamless junction between the lid and the cheek.

How Soon Can One Return to Work?

These operations may be done in either the office operating room or the hospital outpatient surgery suite.  Patients with more sedentary office type jobs may often return to work within the week and resume normal activities within two weeks.
